Duties
- Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support using tools like Hoyer lifts and other assistive devices
- Monitor patient health by observing vital signs such as blood pressure, pulse, and temperature; document medical records accurately using Electronic Health Record (EHR) systems while maintaining HIPAA compliance
- Administer medications safely following prescribed instructions and ensure proper medication documentation
- Support clients with specialized care needs such as Alzheimer’s, dementia, memory care, hospice, and catheter care
- Prepare nutritious meals and assist with feeding while adhering to dietary restrictions and medical guidelines
- Perform light housekeeping tasks including cleaning, laundry, and maintaining a safe environment for clients
- Document all care activities thoroughly in medical records and EMR systems; communicate effectively with healthcare teams regarding patient status and progress
Skills
- Proven experience in home health, assisted living, nursing homes, or long-term care environments
- Strong knowledge of medical terminology, medication administration, and patient care protocols
- Experience working with individuals with developmental disabilities or disabilities requiring specialized support
- Ability to perform patient observation and recognize changes in condition promptly
- Skilled in operating EHR/EHR systems for accurate medical documentation and record keeping
- Certification in CPR and first aid; additional certifications such as Alzheimer's or dementia care are highly valued
- Excellent communication skills to collaborate effectively with clients, families, and healthcare professionals
