Adult Support Specialist/Integrated Care Manager for ACT- Serving Butler County or Warren and Clinton Counties
As part of the recent merger of Best Point and Butler Behavioral Health, we are excited to announce a new opportunity for an Adult Support Specialist!
Butler Behavioral Health has been providing high quality behavioral health services to our community since 1948. Together Butler Behavioral Health and Best Point will continue to value the unique characteristics, individual strengths and a holistic understanding of every client and their resources, to develop personalized treatment plans with focused objectives that result in “small steps” towards changing their lives.
Health Now is a community-based program serving adults with severe and persistent mental illnesses. Services are provided in a holistic manner assuring that the mental and physical needs of the clients are addressed. We are seeking an Adult Support Specialist/ICM to join our ACT team. Our ICM will provide advocacy and support towards the ultimate goal of promoting health and wellness, and enhancing independent living.

Responsibilities
- Recognize, respect and value the role and expertise of the client/family in healthcare decisions.
- Connect client/family to other members of healthcare team through face to face encounters
- Educate clients, family and other providers about healthcare self-management and recovery.
- Provide health promotion, wellness and prevention interventions.
- Link and arrange access to benefits and financial counseling, transportation, home care, and access to social services, peer support and treatment services ensuring that effective connections have been made.
- Implement focused, brief interventions to engage clients and increase their desire meet treatment target
- Teach and practice targeted skills in order to reach goals
- Implement activities to increase knowledge of the most effective interventions
- Associate or higher degree
- Prefer two years of experience working in behavioral health or related field
- An effective, active listener ,with excellent problem solving and communication skills
- Foster and value an environment of diversity
- Demonstrate flexibility and ingenuity to assure that clients receive needed services
- Valid driver's license with a clean driving record
