Hospice - Master's Social Worker
Job Type: Part time

Best Care Hospice is looking for a LMSW or LBSW Social Worker to assist our clients in a professional, compassionate and personal manner. We are seeking a social worker who is willing to work with individuals in various ways. Best Care Hospice is committed to providing services that help people embrace their lives. We are seeking a LMSW or LBSW social workers who are willing to work with individuals in various ways with prior hospice experience.
Requirements:
- Experience working with hospice care/home care
- Experience providing individualized care for clients with all social work needs.
- Ability to work as part of a team for IDGs
- High School Diploma (or equivalent) required, and LMSW
- Valid driver’s license, car insurance and reliable transportation required.
Responsibilities:
- Provide support for patients, families, and friends.
- Collaborate with other clinicians within our team.
- Join IDG meetings with the hospice team to collaborate on patient care.
- Visit patients as needed and provide optimal services.
- Through documentation of all visits and services provided in a timely manner
