Best Care Hospice is looking for a full time hospice nurse case manager for patients in Genesee and surrounding counties. Flexibility in hours, for admissions, revisits and discharges needed.
Responsibilities:
- Conduct comprehensive patient assessments and develop individualized care plans
- Administer medications and treatments as prescribed by physicians
- Provide skilled nursing care to patients in various settings, including homes and hospice facilities
- Monitor and manage symptoms, pain, and comfort levels of patients
- Educate patients and their families on disease management, treatment options, and end-of-life care
- Collaborate with interdisciplinary healthcare team members to ensure holistic care for patients
- Document patient care activities and maintain accurate medical records
Experience:
- Minimum of 2 years of experience as a registered nurse in a clinical setting
- Experience in hospice or palliative care is strongly preferred
- Proficiency in patient assessment, including physical, emotional, and psychosocial aspects
- Knowledge of tube feeding procedures and management
- Ability to provide critical care interventions when necessary
- Familiarity with primary care principles and practices
- Understanding of medical coding and documentation requirements
- Strong knowledge of medical terminology and healthcare procedures
- Certification in Basic Life Support (BLS) is required
