We are seeking a dynamic and compassionate Nurse Practitioner specializing in Geriatric Behavioral Health to join our dedicated healthcare team. Responsibilities

• Conduct thorough psychiatric and medical assessments for geriatric patients, including mental health evaluations and medication reviews.
• Develop and implement individualized treatment plans that incorporate behavioral health strategies, medication management, and patient education.
• Collaborate closely with interdisciplinary teams including physicians, social workers, therapists, and family members to coordinate holistic care.
• Administer or oversee administration of treatments such as Botulinum toxin therapy for specific conditions when appropriate.
• Manage complex cases involving dementia, memory care, and behavioral disturbances, ensuring safety and optimal outcomes.
• Utilize electronic health record (EHR) systems such as Medsphere document patient encounters accurately and efficiently.
• Provide telehealth services to expand access to care for seniors in various settings, including assisted living facilities and home care environments.

Skills

• Proven experience working with older adults in geriatric or behavioral health settings; experience with dementia and Alzheimer's care is highly valued.
• Strong knowledge of medical documentation standards, CPT coding, ICD-10 coding systems, and HIPAA compliance.
• Skilled in performing clinical procedures such as venipuncture, IV infusion, suturing, catheterization, spinal taps, and sterile processing techniques.
• Ability to perform comprehensive patient assessments including vital signs, physical examinations, and physiology knowledge.
• Experience managing acute pain cases, medication administration including psychopharmacology, and utilizing telehealth platforms effectively.
