Are you an experienced Social Worker looking for a rewarding career in Providence? Join our exceptional team at Berkshire Place Nursing, conveniently located at 455 Douglas Ave, Providence, RI 02908. We are seeking a dedicated Director of Social Services to help us deliver outstanding care to our residents.
Your Role as Director of Social Services includes but is not limited to:
Preparing and maintaining current case records for each resident including the nature of social problems and notes from conferences held with resident, family or significant other.
Performing a variety of services for meeting the psychosocial needs of residents and their families such as: assisting with initial adjustment and subsequent changes, crisis management, providing financial counseling and assistance and coordinating in house room transfers
Acts as a resource to other Social Workers
Assess residents’ psychosocial needs and create individualized care plans to support their well-being.
Act as a liaison between residents, families, and the healthcare team to ensure open communication and address concerns.
Facilitate discharge planning and connect residents with community resources and services.
Organize family meetings and support groups to address emotional and social needs.
Ensure compliance with state and federal regulations regarding social services in long-term care facilities.
Advocate for residents’ rights and provide emotional support during transitions and challenges.
What You Need:
Master's degree in Social Work (MSW) from an accredited institution.
Minimum of 2 years of long term care/ skilled nursing experience required.
Strong knowledge of social work practices, ethics, and regulations.
Exceptional leadership and interpersonal skills.
Excellent communication and problem-solving abilities.
Dedication to diversity, equity, and inclusion.
Proficiency in utilizing technology for data analysis and reporting
