Nursing Care Coordinator – Berks Community Health Center

Berks Community Health Center is seeking a Nursing Care Coordinator to lead a clinical support team and help deliver high-quality, patient-centered care in a fast-paced Federally Qualified Health Center (FQHC) environment. This role works closely with providers, nurses, medical assistants, and patient services staff to coordinate care, improve patient outcomes, and support continuity of care across the lifespan.

What You’ll Do

• Coordinate care for patients and serve as the primary liaison between patients and providers
• Lead and support the clinical team to ensure efficient daily operations
• Track referrals, follow-ups, and care plans to support quality and PCMH standards
• Provide patient education, triage support, nurse visits, and clinical assessments
• Collaborate with providers and community resources to improve patient outcomes
• Support quality, safety, and compliance initiatives in an FQHC setting

Qualifications

• Registered Nurse (RN) required
• Care coordination, primary care, or FQHC experience preferred
• Strong communication, leadership, and organizational skills
• Passion for patient-centered, community-focused care

Job Type: Full-time

Pay: $75,000.00 - $85,000.00 per year
