Hospice Social Worker (LCSW) – Full-Time
Berkley Palliative Care & Hospice is excited to announce an exciting opportunity to join our growing team! We are currently seeking a Hospice Social Worker (LCSW) who will be responsible for providing quality and safe delivery of Medical Social Work services that reflect the philosophy and standards of Berkley Hospice.
This role provides medical social work services in a hospice setting, supporting patients and families with compassionate, high-quality end-of-life care.
Key Responsibilities
- Conduct psychosocial assessments for hospice patients and families
- Develop and implement individualized plans of care (POC)
- Provide counseling, emotional support, and education related to hospice care
- Coordinate with nurses, physicians, therapists, and IDG team members
- Connect patients with community resources and support services
- Participate in IDG/IDT meetings and care conferences
- Maintain timely, accurate clinical documentation (WellSky Consolo - EMR)
- Support quality improvement (QAPI) initiatives
Qualifications
- LCSW
- Minimum 2 years of experience in hospice, home health, or healthcare social work
- Knowledge of Medicare hospice regulations and interdisciplinary care model preferred
- Strong communication, organization, and documentation skills
- Valid driver’s license, auto insurance, and reliable transportation

Pay: $70,000.00 - $78,000.00 per year
