What are the responsibilities and job description for the Transitional Care Nurse Coordinator and Trauma Nurse Coordinator, BTMC Emergency Room (Full Time) position at Benefis Health System?

Responsible for overall assessment, planning implementation, and evaluation of transitional patient care. Responsible for implementing the Nursing Service mission, vision, values and overall strategic plan, clinical management, patient care, and quality management on the unit. Promotes the hospital standards and creates a patient centered caring environment while addressing and meeting the induvial needs of the patient. Supports patients and their families as they move through each level of care from preadmission through discharge. Fosters an environment of collaboration and respect with physician colleagues, nurses, and other healthcare team members.
Initiates and completes the required MDS documentation (not required for submission per CAH State Operation Manual) for swing bed residents/patients and ensures the documentation accurately describes and reflect resident status. Clearly, concisely, and effectively communicates this resident information to the other members of the health care team for use in care planning. Coordinates weekly Care Plan meetings with interdisciplinary team.
Responsible for coordination and facilitation of Trauma Performance Improvement. Trauma Performance Improvement includes all areas of disciplines of the hospital that participate in care of the trauma patient. Performs data checks for validity/reliability and submitting data to the state central site quarterly and to the National Trauma Data Bank on an annual basis. Provides coordination and facilitation of performance improvement data collection, analysis, trending, and benchmarking of data outcomes. Uses Trauma Registry data (state, regional and local) to drive PI projects. Facilitates multi-disciplinary trauma committee meetings. Collaborates with Benefis Health System Trauma Coordinator for overall trauma oversight, education, and training.
Demonstrates the ability to deal with pressure to meet deadlines, to be accurate, and to oversee and handle constantly changing situations. Demonstrates the ability to deal with a variety of people, deal with stressful situations, and handle conflict.
Will perform all job duties or job tasks as assigned. Will follow and adhere to all requirements, regulations and procedures of any licensing board or agency. Must comply with all Benefis Health System’s organization policies and procedures.

Education/License/Experience Requirements:
- Current licensure as a Registered Nurse in the State of Montana.
- Master’s degree in healthcare is preferred
- A minimum of three (3) years as a Registered Nurse; Case Management experience is strongly preferred
- Demonstrated knowledge of utilization management principles
- Marketing or Sales background in a health care setting strongly preferred
- Must be able to travel within the region
- Valid Driver’s License, vehicle insurance, and dependable transportation
- BLS, ACLS, PALS required or to be obtained within six (6) months after hire
- TNCC (Trauma Nursing Core Course) completed
- Certification in Emergency Nursing preferred (CEN)
- Minimum of two (2) years’ experience in the Emergency Department or Critical Care Unit.
