Meadow Ridge is the premier life care retirement community in Fairfield County, located on 136 acres in beautiful Redding, Connecticut. In this unique and enriching environment, we offer Independent and Assisted Living apartment homes, as well as a skilled Health Center where we provide our residents with quality care and services.
We are currently seeking a Coordinator to provide administrative support for our Social Services department in Skilled Nursing.

15 hours a week; flexible afternoon scheduling options possible
$23/hr
Primary responsibilities of the role:
- Assist the Licensed Social Worker in gathering psychosocial assessment information upon admission and as needed.
- Support development and implementation of individualized care plans.
- Participate in interdisciplinary team meetings and care conferences.
- Provide emotional support to residents and families.
- Assist with discharge planning, including coordination of community resources and referrals.
- Help residents adjust to long-term care placement.
- Document resident interactions and interventions in the medical record accurately and timely.
- Investigate and report concerns, grievances, or signs of abuse/neglect according to facility policy.
- Assist with advanced directives as directed.
- Maintain confidentiality and comply with HIPAA regulations.
- Support residents’ rights and advocate for resident needs.
- Assist with organizing resident and family meetings.
- Perform administrative duties such as filing, tracking referrals, and maintaining social service documentation.
The successful candidate will have:
- Experience in healthcare, long-term care, or social services preferred.
- Knowledge of Medicare/Medicaid and long-term care regulations is a plus.
- Understanding of psychosocial needs of geriatric and disabled populations.
- Strong communication and interpersonal skills.
- Ability to handle sensitive and confidential information.
- Organizational and documentation skills.
- Compassionate, patient-centered approach.
- Ability to work collaboratively within an interdisciplinary team.
- Basic computer proficiency and electronic medical record (EMR) experience.
