General Job Description:
Responsible for service coordination and outreach services to patients * Provide intensive home visitation services for pregnant and parenting families * Promote the health and well-being of clients by informing potential consumers about available services * Provide transportation services to patients of Ben Archer Health Centers when appropriate and available * High School graduate or GED is required; college degree in Social Work, Early Childhood Development, Marriage and Family Counseling, Infant Family Studies or a related field is preferred * Two years community outreach experience in a social services function and experience with the early childhood population is preferred * Must have current NM Driver’s License and a clean driving record * Must pass a background check * Ability to speak English and Spanish is preferred * Maintains appropriate confidentiality * Must possess and maintain a clean driving record
