Bellomo and Associates is seeking a compassionate, driven Life Care Planning Social Worker to join our team in York, PA. This role is ideal for someone passionate about elder law, advocacy, and supporting families through life’s transitions.
Key Responsibilities:
- Conduct comprehensive assessments to create personalized Life Care Plans addressing social, emotional, medical, and financial needs.
- Provide counseling and emotional support to clients and families navigating aging and legal challenges.
- Educate clients on elder law issues and available legal options.
- Advocate for clients' rights and connect them with community resources, including healthcare, financial support, and caregiver services.
- Maintain accurate, confidential records in accordance with legal and ethical standards.
- Deliver exceptional client service both in-person and by phone.
- Collaborate closely with attorneys and team members to ensure coordinated, comprehensive care.
- Participate in community outreach and education to raise awareness of Life Care Planning services.
- Uphold strict confidentiality and professionalism in all client matters.
- Address client concerns proactively, escalating when necessary.
- Communicate with cultural sensitivity and respect for individual differences.

Qualifications:
- Bachelor Degree in a related field of Social Work, Healthcare, Gerontology, Case Management, Healthcare Administration or related field
- Preferred: Be a Licensed Bachelor Social Worker, Licensed Social Worker or Licensed Clinical Social Worker
- Preferred: Experience with elder care, healthcare systems, or legal services
Job Type: Full-time
Expected hours: 37.5 – 40 per week
