Position Overview
As an RN Assessor, you will conduct in-home evaluations for long-term care insurance eligibility. You’ll assess clients’ physical, cognitive, and functional needs while helping families secure the benefits and support services they need to remain safely at home.
Responsibilities
- Conduct in-home client assessments
- Evaluate Activities of Daily Living (ADLs) and cognitive status
- Complete clear and accurate clinical documentation
- Submit assessment reports within required timeframes
- Collaborate with insurance providers and care teams
- Support clients and families throughout the assessment process
Qualifications
- Active Pennsylvania RN License
- Strong clinical assessment and documentation skills
- Excellent communication and organizational abilities
- Compassionate, professional bedside manner
- Reliable transportation for local travel
