Role Description
This is a full-time on-site role for a Case Manager located in Plano, TX. The Case Manager will be responsible for providing comprehensive case management services alongside the Senior Case Managerincluding developing personalized care plans, facilitating access to resources and programs, and offering emotional support. Daily tasks include conducting assessments, coordinating services, maintaining accurate records, and working closely with residents to achieve their personal goals. The Case Manager will also collaborate with other staff and community partners to ensure holistic support for each mother.

Qualifications
- Experience in case management, social work, or related field
- Strong assessment and care planning skills
- Ability to facilitate access to resources and programs
- Excellent organizational and record-keeping abilities
- Strong communication ,conflict resolution and interpersonal skills
- Compassionate and empathetic, with a commitment to supporting mothers in need
- Bachelor's degree in Social Work or a related field preferred
- Experience working in a residential or community outreach program is a plus
- Must be passionate about working with mothers and babies
