We are looking for a reliable RN (Registered Nurse) or LPN (Licensed Practical Nurse) to care for Hospice patients in a professional and considerate manner.
We are currently looking for a weeknight on-call RN or LPN.
Responsibilities
- Implements current nursing practice following a patient assessment and an approved plan of care initiated by the registered nurse.
- Provide accurate and timely documentation of patient services to reflect the plan of care.
- Assess and provide patient and family/caregiver education and information pertinent to diagnosis and plan of care. Collaborates with RN case manager(s).
- Participates in coordination of hospice services, appropriately reporting the identified needs for hospice interdisciplinary group members. Examples include hospice aide, OT, PT, MSW, ST, Dietitian or Supervisor.
- Uses equipment and supplies effectively and efficiently.
- Provides appropriate pain/symptom management. Evaluates patient’s response to treatments/medications.
- Assisting with evaluating quality assessment performance improvement (QAPI) plans and participates in QAPI teams and activities, as needed.
- Performs other duties as assigned by the registered nurse.
Skills
- Proven experience as RN or LPN.
- Ability to undertake a variety of diverse care tasks and work in a multidisciplinary team
- Familiarity with health, safety and sanitation standards and procedures
- Understanding of confidentiality obligations and nursing best practices
- Excellent knowledge of medical and hospital terminology
- Good knowledge of MS Office and data entry
- Excellent communication and interpersonal skills
- Compassionate and able to handle stress
- Successful completion of LPN license program is a must

Requirements
- Valid RN licensure or LPN Licensure
- CPR certification required
- Hospice experience preferred
