What are the responsibilities and job description for the Case Manager Next Door (Full Time Position) position at Bell Socialization Services?
The Case Manager position is responsible for assisting consumers with housing needs and goal setting, proper preparation and maintenance of required documentation and records as well as close coordination with other services and agencies. Because of the nature of this Agency, it is essential that the Rental Assistance Case manager believe in the philosophy of the Agency as well as be able to develop a rapport with program participants. Assess individual and family housing needs and make referrals to community services. Assess children’s needs and make appropriate referrals. Facilitate the program goals of assisting residents with finding safe, affordable and permanent housing in the community through the program. Determine which services an individual or family is eligible to receive from the program. Assist consumers with determining goals necessary for finding and maintain housing, including tenancy and budgeting skills. Provide support services to help consumers maintain their housing.
