As a Staff Supervisor, you will play a vital leadership role in supporting adults enrolled in the One Care program by overseeing a team of Care Coordinators who provide high-quality, person-centered care management services. You’ll help ensure individuals with medical, behavioral health, and/or substance use needs receive compassionate support, meaningful community connections, and coordinated care that promotes stability, independence, and overall well-being.

What You’ll Do

  • Provide supervision, guidance, and leadership to a team of Care Coordinators to ensure high-quality service delivery
  • Ensure staff meet all contractual and program requirements, including outreach frequency, care plan completion, documentation, and productivity expectations
  • Support individuals served through telephonic, virtual, in-person, and community-based engagement as needed
  • Promote person-centered care by ensuring individuals are connected to appropriate healthcare, behavioral health, housing, employment, and community resources
  • Build and maintain strong relationships with providers, stakeholders, funding sources, and community partners
  • Facilitate regularly scheduled care team meetings and support interdisciplinary collaboration
  • Provide coaching, mentorship, and constructive feedback to staff while fostering accountability and professional growth
  • Identify safety concerns and support crisis intervention efforts in collaboration with program leadership and emergency services when needed
  • Support transitions from higher levels of care back into the community, including hospitalizations, emergency rooms, and detox settings
  • Ensure timely, accurate, and compliant documentation in accordance with program and funder requirements
  • Participate in leadership meetings, trainings, field shadowing, and ongoing professional development
  • Respond to emails and phone communications within one business day
  • Uphold agency policies, procedures, and BHN’s mission-driven values in all interactions

Qualifications

High School Diploma or GED required; Bachelor’s Degree in a related mental health field preferred

Minimum of 2 years’ experience working in a human service field required

Previous Management Or Supervisory Experience Preferred

Experience working with individuals with behavioral health and/or substance use disorders strongly preferred

Bilingual (Spanish/English) preferred

Knowledge of mental health, substance use, care management, and the intersection of medical and behavioral health systems

Knowledge of community resources and social determinants of health

Strong verbal and written communication skills

Familiarity with electronic health records (EHRs), email systems, and basic computer technology

Valid driver’s license and reliable, legally registered vehicle available during working hours required

Schedule: Full-Time Hybrid- Monday - Friday 8am-4:30p

presence in the office and in the community is required.

Salary: $54k - $69K
