What You Will Do

As a Care Navigator, you’ll work directly with individuals involved with the criminal justice system who may be experiencing behavioral health and/or substance use challenges. You’ll help individuals overcome barriers to care, connect with community resources, and build the support they need to achieve greater stability and independence.

  • Provide in-person, telephone, and telehealth care management and support to individuals on your caseload
  • Meet with individuals in jails, hospitals, substance use treatment facilities, and other 24-hour settings to support discharge planning and connection to community services
  • Serve as the primary point of contact for individuals on your caseload and help coordinate their care and supports
  • Complete biopsychosocial assessments, individualized support plans, safety plans, and disenrollment plans in collaboration with individuals, providers, and natural supports
  • Build collaborative relationships with probation and parole officers, court personnel, correctional staff, primary care providers, behavioral health and substance use providers, and other members of an individual’s support network
  • Help individuals access community resources, including health insurance, food and nutrition assistance, housing, financial assistance, utilities, education, employment, and legal resources
  • Provide crisis support in collaboration with the Clinical Supervisor and Emergency Services Program
  • Participate in a 24/7 on-call rotation to support individuals during times of crisis
  • Maintain professional and collaborative relationships with providers, community partners, and other supports
  • Attend required meetings, trainings, and other program activities
  • Complete required documentation and other duties as assigned

Who You Are

  • High school diploma required
  • Community Health Worker certification and/or bachelor’s degree preferred
  • 1–2 years of experience in behavioral health, substance use, corrections, human services, or a related field required
  • Knowledge of mental health and substance use disorders and experience connecting individuals to community resources
  • Experience working with justice-involved individuals and/or justice system partners preferred
  • Knowledge of care management and community-based services
  • Strong communication, relationship-building, and collaboration skills
  • Ability to develop therapeutic and supportive relationships with individuals served
  • Spanish/English bilingual skills preferred
  • Lived experience with the criminal justice system and/or recovery from behavioral health or substance use challenges preferred
  • Comfortable using computer systems and electronic documentation
  • Valid Massachusetts driver’s license required
  • Reliable transportation required; vehicle must be properly registered and inspected in accordance with Massachusetts requirements

Pay: $21.50- $25.27
