Overview:
We are seeking a compassionate and dedicated Case Manager to join our behavioral health team. This position will serve clients experiencing Substance Use Disorders (SUD) and mental health challenges, providing comprehensive support to help them achieve stability and wellness. The ideal candidate will have experience in behavioral health, crisis intervention, and client advocacy, and will be comfortable conducting occasional home visits as needed.
Key Responsibilities:
- Provide case management services including intake assessments, care planning, coordination, and follow-up.
- Conduct regular client evaluations to monitor progress and adjust care plans accordingly.
- Collaborate with healthcare providers, treatment teams, social workers, and community agencies to ensure integrated care.
- Support clients in addressing mental health and/or substance use issues through counseling, referrals, and service coordination.
- Supervise and mentor clients engaged in workforce development and job-readiness programs.
- Assist with arranging transportation and childcare for clients as needed to support treatment, employment, and recovery goals.
- Manage and monitor child welfare cases, ensuring the safety and well-being of children involved.
- Conduct home visits as required to assess client needs and ensure follow-through on care plans.
Qualifications:
- Associate’s degree in social work, psychology, human services, or a related field is required; Bachelor’s degree is preferred.
- CDCA, LCDC, or QMHS, certifications are required or ability to obtain within 3 months of starting position.
- Previous experience in case management, particularly in areas such as SUD treatment, mental health, child welfare, addiction counseling, Medicare services, or workforce development.
- Strong knowledge of crisis intervention strategies and behavioral health systems.
- Ability to work independently, manage a diverse caseload, and perform home visits.
- Reliable transportation and valid driver’s license required to support client home visits and transportation coordination.
