What are the responsibilities and job description for the Targeted Case Manager position at Behavioral Health Advocacy Services?
The Targeted Case Manager (TCM) provides person-centered, community-based mental health case management services to adults with serious and persistent mental illness (SPMI) or children with serious emotional disturbance (SED). The TCM supports clients in achieving recovery goals, improving stability, and maximizing independence. This role involves assessment, planning, service coordination, advocacy, monitoring, and crisis support in alignment with Minnesota Department of Human Services (DHS) standards.
Essential Duties & Responsibilities1. Client Assessment & Service Planning
- Conduct comprehensive assessments required by DHS, including functional, and risk assessments.
- Partner with clients to develop an Individual Community Support Plan (ICSP) or an Individual Family Community Support Plan (IFCSP) that identifies services and supports necessary to promote independence, recovery, and improved family functioning.
- Treat all clients with dignity and respect, delivering services that honor each individual’s diagnosis, age, gender, culture, and preferences.
- Review and update service plans regularly or when client needs change.
2. Service Coordination & Integrated Supports
- Coordinate integrated services and supports necessary to assist each client as they work toward their recovery goals.
- Assist clients in accessing mental health services, medical care, housing resources, financial assistance, transportation, vocational supports, and other essential community resources.
- Assure that services are implemented as documented in the ICSP or IFCSP and that they are continuously monitored and evaluated for effectiveness.
- Facilitate referrals, maintain collaborative relationships with community partners, and ensure continuity of care.
3. Collaboration, Education & Advocacy
- Advocate for client needs across service systems, promoting access to appropriate care and resources.
- Provide collaboration and education to significant others and family members—as requested by the client—who are involved in supporting their recovery.
- Empower clients to build independence, self-advocacy skills, and knowledge of available community supports.
4. Monitoring & Documentation
- Maintain timely, comprehensive documentation within the electronic health record (EHR) in accordance with DHS and agency standards.
- Track progress toward service plan goals through regular monitoring and client contact.
- Assure that all changes in client status or needs are clearly communicated through documentation.
- Maintain compliance with all DHS contact frequency and documentation requirements.
5. Crisis Intervention
- Provide crisis intervention and coordinate access to emergency services when clients experience instability or safety concerns.
- Support clients in developing and utilizing crisis and safety plans.
6. Professional Standards & Team Participation
- Maintain client confidentiality following HIPAA and agency policies.
- Participate in staff meetings and required trainings, supporting continuous learning and professional development.
- Uphold professional boundaries and ethical service delivery at all times.
Required Qualifications
- Bachelor’s degree in social work, psychology, human services, or a related behavioral health field OR qualifications meeting DHS Mental Health Practitioner or Case Management eligibility.
- Valid driver’s license, reliable transportation, and proof of insurance.
- Ability to pass a DHS background check.
- Strong communication, problem-solving, and organizational skills.
Preferred Qualifications
- 1-2 years of experience providing case management to individuals with SPMI or children with SED.
- Working knowledge of Minnesota DHS mental health service requirements.
- Experience with EHR systems and community mental health service coordination.
- Multilingual skills and cultural competence.
Work Environment
- Services are delivered in office, community, home, and virtual settings, depending on client needs.
- Position requires regular travel to client homes and community locations.
- Occasional evening or weekend hours may be needed.
Physical Requirements
- Ability to sit, stand, walk, and participate in community-based work.
- Ability to lift up to 20 pounds occasionally.
- Ability to navigate various community environments safely
Compensation & Benefits
- Competitive hourly wage based on experience $22 to $28/per hour
