Behavioral Crossroads, LLC is seeking a part-time Behavioral Health Home Program Supervisor (LPN or RN) to join our team! The position is in-office in Egg Harbor Township, part-time [schedule can be flexible but would be during the day (could be as early as 7am or as late as 4pm)] and includes paid time off. The pay range for this position is between $35-40 per hour.
Job Responsibilities include, but are not limited to:
· Managing the Behavioral Health Home team with care coordination and care transitioning.
· Participating in daily rounding meetings.
· Working with APNs to complete health assessments on clients of Behavioral Health Home.
· Managing the screening of potential clients for eligibility for Behavioral Health Home.
· Participating in the development of wellness programming for program participants.
· Conducting outreach and engagement activities with program participants.
· Collaborating with providers in the community who are involved with clients in the Behavioral Health Home program.
· Coordinating the clients' care with community treatment providers, and overseeing care planning.
· Assessing program participants' wellness needs, and conducting wellness and engagement activities.
· Participating in treatment planning with the behavioral health team.
· Assisting with required data collection on clients enrolled in the program.
· Developing and implementing WRAP services to program participants.
· Working with Transportation department to coordinate transporting consumers to needed appointments and services.
· Working closely with billing and finance to ensure coordinated billing process.
· Ensuring full compliance with regulatory requirements, policies, and procedures.
· Providing coverage and support for Behavioral Crossroads Mental Health groups.
· May carry a BHH and partial care client caseload, and administer behavioral health medication injections to program participants as needed.
Well-qualified candidates should have a NJ LPN or RN license. Bachelor's Degree's preferred. Related experience in a behavioral health setting preferred, but not required.
