The Wesley Rehabilitation Center at Beechwood Homes is looking for one dedicated and motivated full-time RN for Short-term rehabilitation nursing. This position is full-time with 12-hour shifts. I. JOB SUMMARY: Assist the R.N. Neighborhood Manager and Assistant Director of Nursing in the responsibility for total Patient care and with management of the Neighborhood nursing team.
II. ESSENTIAL COMPETENCIES:
1. In absence of RN Neighborhood Manager, is responsible for total patient care within the nursing Neighborhood during the shift.
2. Assess Patients’ needs, condition, and care. In absence of RN Neighborhood Manager, develops a 24-hour Patient Care Plan for each Patient as necessary. Reviews the plan on a regular basis and attends Care Plan Meetings. Integrates recommendations of other departments into 24-hour Care Plan. Assist in the coordinating of nursing care to ensure 24-hour Patient Care Plan is implemented. Evaluates outcome of Care Plan.
3. Performs and/or supervises patient care including personal hygiene, administration of medications, giving of treatments, and other professional nursing services as ordered or required. Supervises and performs charting.
4. Makes frequent rounds to ensure appropriate Patient care and staff performance.
5. Accompanies physicians on round, updating him/her on Patients progress and plan of care. May assist with examinations and treatment. Supervises and transcribes physicians orders.
6. Supervises and provides emergency treatment when required including notifying the physician, taking and carrying out physician’s orders, notifications of families as necessary, and required documentation
7. Works with Quality Assurance in mind at all times. Supervises the correct use of equipment, makes out incident/accident reports as necessary and proceeds with follow-up appropriately.
8. Coordinates nursing services and other health related services to improve and maintain the quality of patient care.
9. Assures Patient’s right to fair and equitable treatment, self-determination, individuality, privacy, property and civil rights, including the right to file complaints, are followed.
10. Provides, participates in, or arranges for Patient and/or family counseling.
11. Is aware of Patients’ spiritual and cultural needs and arranges for these to be met.
12. Supervises and plans assignment of Patients care to Team Leaders and CNA’s.
13. Leads staff conferences to promote initiative and solve Patient care problems.
14. Issues staff member “counseling” or Disciplinary Action Reports as needed and makes referrals for remedial training as needed.
15. Attends facility committee meetings deemed necessary by Nursing Management.
16. Attends and participates in inservice education classes and on-the-job training programs as required. Attends and participates in workshops, seminars, etc. as directed.
17. Maintains CONFIDENTIALITY of all pertinent Patient care and facility information.
18. Completes accurate M.D.S., FIM as per policy. Uses e-mail as a communication tool.
19. CPR-certified and participates in “codes” (CPR situations).
III. OTHER RELATED COMPETENCIES:
1. Provides professional nursing services. Assist RN Neighborhood Manager and Assistant Director of Nursing with Physical assessments, initial and ongoing interdisciplinary Patient care planning, assurance of care plan implementation. Neighborhood management, and Staff Development efforts.
2. Carries out administrative and Patient care duties as directed by RN Neighborhood Manager and Assistant Director of Nursing.
3. Supervision of all other professional and para-professional nursing staff.
4. Completes or assures completion of Daily Patient Status Summary.
5. Follows all established safety procedures and precautions when operating equipment. Reports equipment malfunctions/breakdowns and unsafe/hazardous conditions to plant operations and informs supervisor as soon as possible.
6. Performs other duties as assigned by department director.
IV. WORKING CONDITIONS:
Well-lighted, well-ventilated rooms and halls. Sits, stands, and walks most of the day on nursing Neighborhood. Lifts, moves, and supports Patients and equipment (with assistance) up to 250 lbs. Must deal with Patients and visitors on a personal basis under all conditions. Must handle emergency situation, give emergency care, and give reassurance to Patients and families. Must share weekend and holiday work.
V. PERSONAL QUALIFICATIONS:
Graduate of an approved School of Nursing. Current registration in New York State. Membership in professional nursing organization recommended. Six (6) months experience in medical-surgical nursing preferred. Previous experience with aging Patients desirable. Experience in supervisory capacity preferred. Sincere desire to work with the aging and infirm. Must have thorough knowledge of medical, social, rehabilitative, and psychological needs of Patients in this type of facility. Able to supervise and instruct others. Well-groomed, courteous, tactful, and pleasant when dealing with Patients. Level-headed in emergency situations. Acute sense of responsibility. Must possess the spirit of cooperation and enthusiasm in order to create an atmosphere conducive to rehabilitation and growth. Must be neat in appearance and abide with dress code.
