About the Role:
As a Home Care Physical Therapy Assistant, you will:
- Provide both direct and indirect patient care to facilitate the rehabilitation and restoration of normal body functions.
- Under the direction of a physical therapist, conduct treatment programs based on each patient's plan of care.
- Administer therapeutic modalities and train patients in therapeutic exercises, transfers, and gait.
Responsibilities:
- Physical Therapy Skilled Treatments.
- Client Care Coordination.
- Teaching, Supervision and Counseling Clients and Caregivers.
- Documentation of Interdisciplinary Communication.
- Problem Solving and Critical Thinking Skills.
- Documentation.
- Ability to effectively and safely transfer patients with significant physical and/or mental impairment.
- Time Management
- Assists in Deliver of Plan of Care
- Able to recognize the need for a change in the plan of care and effectively communicating this to the supervising therapist in a timely manner
Competencies Skills:
- Clear Communication Skills Both Written and Verbal
- Able To Keep Confidential Information Regarding Patients, Team Members
- Able To Withstand Crisis Situations
- Has Skills to Provides Customer Service to Patients, Team Members and Visitors
- Proof Of Current/Valid Car Insurance
- Knowledge And Experience with Electronic Health Records
- Experience With Excel, Power Point, Word, Visio, Etc.
- Valid DL With No More Than 3 Moving Violations
- PTA - Physical Therapy Assistant
- BLS - Basic Life Saving certification
Minimum required: Associates Degree
Other Information:Entry: USD $24.62/Hr. Max: USD $38.16/Hr.
