Duties

• Assist clients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and mobility support using equipment like Hoyer lifts and ensuring proper patient handling techniques.
• Monitor and record vital signs, observe patient conditions, and report any changes or concerns promptly to healthcare professionals.
• Prepare nutritious meals tailored to dietary restrictions and preferences while maintaining proper food safety and sanitation standards.
• Administer medications accurately following prescribed care plans and HIPAA regulations to ensure patient safety and compliance.
• Maintain a clean, safe, and organized living environment by performing light housekeeping tasks including cleaning, laundry, and sanitation routines.
• Support clients with specialized needs such as dementia care, Alzheimer’s care, hospice support, or working with individuals with developmental disabilities or autism.
• Document all care activities thoroughly in accordance with care plans and legal requirements to ensure continuity of quality care.

Requirements

• Proven experience in home health services, assisted living, nursing homes, or group home settings with a focus on senior care or long-term care.
• Certification in First Aid and CPR is required; additional certifications such as HIPAA training are preferred.
• Demonstrated ability to work with individuals with disabilities or cognitive impairments such as dementia or autism spectrum disorder.
• Strong observational skills for patient monitoring and recognizing changes in health status.
• Ability to perform heavy lifting safely when assisting patients with mobility using equipment like Hoyer lifts or resident’s .
• Excellent communication skills for collaborating with healthcare teams, residents, families, and social workers.
• Flexibility to work various shifts including days, evenings, weekends or overnight as needed.
