Responsibilities

  • Conduct comprehensive assessments of clients’ needs, including mental health, addiction, child welfare concerns, and housing stability.
  • Develop personalized care plans that incorporate evidence-based interventions such as motivational interviewing, CBT techniques, and crisis management strategies.
  • Provide individual and group therapy sessions to support clients’ emotional regulation, recovery, and overall well-being.
  • Deliver crisis intervention and behavioral health support during urgent or high-risk situations, ensuring client safety and stabilization.
  • Offer addiction counseling and recovery-focused services to promote insight, coping, and long-term stability.
  • Provide consistent case management, including coordinating resources, advocating for client needs, and ensuring follow-up on referrals.
  • Collaborate with community agencies—including Section 8 housing programs, Medicare providers, workforce development programs, and child welfare services—to support comprehensive care planning.
  • Maintain accurate, timely, and compliant documentation of assessments, treatment plans, progress notes, and client interactions in accordance with regulatory standards.
  • Participate in interdisciplinary team meetings to ensure coordinated, client-centered care.

Skills

  • Strong background in addiction counseling, behavioral health services, and evidence-based social work practices.
  • Excellent communication and interpersonal skills, with the ability to motivate and empower clients using motivational interviewing strategies.
  • Demonstrated ability to supervise, train, and develop staff or interns, promoting teamwork and professional growth.
  • Ability to manage multiple cases efficiently while maintaining exceptional attention to detail in documentation and follow-up.
