Responsibilities:
- Provide direct patient care to residents, including assistance with daily living activities, such as bathing, dressing, and grooming
- Take vital signs and report any changes to the nursing staff
- Assist with mealtime and snacks, including feeding when necessary
- Maintain a clean and safe environment, including housekeeping and laundry duties
- Collaborate with the nursing team to develop and implement individualized care plans
- Communicate effectively with patients, families, and healthcare team members
- Perform other duties as assigned by the nursing staff
