The Director of Social Services is responsible for planning, developing, organizing, evaluating, and directing the overall operation of the Social Services Department in accordance with current federal, state, and local standards, guidelines, and regulations. This position ensures residents receive the highest level of psychosocial support and that their social, emotional, and discharge planning needs are met.
Essential Job Duties & Responsibilities
- Assess the social and emotional needs of new and existing residents.
- Develop individualized care plans in coordination with interdisciplinary teams.
- Advocate for residents’ rights and help ensure a positive quality of life.
- Support residents and families during admissions, transitions, care conferences, and discharge planning.
- Serve as a liaison between residents, families, facility staff, and community agencies.
- Coordinate services such as counseling, legal aid, and financial assistance as needed.
- Document social histories, assessments, progress notes, and care plans in compliance with regulatory standards.
- Ensure compliance with Medicare/Medicaid regulations, HIPAA, and state long-term care requirements.
- Supervise and train social services staff (if applicable).
- Participate in facility surveys (inspections) and assist in developing plans of correction when necessary.
- Organize support groups or educational programs for residents and families as needed.
- Bachelor’s degree in Social Work (BSW) required; Master’s degree (MSW) preferred.
- Current Social Work license or certification as required by state regulations.
- Minimum of 2 years’ experience in a long-term care facility or similar healthcare setting.
- Strong understanding of long-term care regulations, discharge planning, and resident advocacy.
- Excellent communication, interpersonal, and organizational skills.
- Compassionate, patient-centered, and culturally sensitive approach
