**Responsibilities**

• Provide treatment to residents under the direction of a licensed Physical Therapist
• Assist in carrying out individualized therapy plans to improve mobility, function, and quality of life
• Document services accurately per company, state, and federal guidelines
• Collaborate closely with PTs, other rehab staff, nursing, and interdisciplinary team members
• Educate residents and families on home exercises and functional strategies
• Track patient progress and communicate updates to the therapy team
