**Responsibilities**
- Provide treatment to residents under the direction of a licensed Physical Therapist
- Assist in carrying out individualized therapy plans to improve mobility, function, and quality of life
- Document services accurately per company, state, and federal guidelines
- Collaborate closely with PTs, other rehab staff, nursing, and interdisciplinary team members
- Educate residents and families on home exercises and functional strategies
- Track patient progress and communicate updates to the therapy team
