Description: Full-time Transition Coordinator needed for Bedford High School beginning August 2026. Ideal candidate will have three or more years experience working with young adults (ages 14-21) with Individual Education Plans and transition services.Job Summary: To plan, coordinate and implement the transition programs and services of special education students. The Transition Coordinator will monitor the development and implementation of individual transition plans and goals to ensure that each identified student successfully transitions into post-secondary opportunities. The Transition Coordinator will develop, maintain and oversee both school based and community based jobs and supervise job coaches assigned to each.Qualifications: Bachelor's degree in Special Education, Vocational Education, Vocational Rehabilitation, Social Services or comparable area.
