As a residential home health and hospice registered nurse case manager, you'll be responsible for:

- Developing and implementing individualized care plans that address the physical, emotional, and social needs of patients
- Collaborating with healthcare professionals to ensure seamless care transitions and comprehensive support
Requirements
To be successful in this role, you'll need:
- A valid RN license in the state where you'll be working
- Graduation from an approved school of practical nursing
- At least one year of RN experience and demonstrated clinical competence
- Experience working with tablets, mobile phones, and EMR software
