Job Summary: We are seeking a skilled Community Health Representative to join our team. As a Community Health Representative, you will work closely with community members to promote health and wellness.
Responsibilities:
- Develop and implement community outreach programs to educate residents about healthy living and disease prevention.
- Collaborate with local healthcare providers to ensure seamless care coordination.
- Provide guidance and support to individuals on healthy lifestyle choices.
Requirements:
- A minimum of 2 years of experience working in the healthcare setting or completion of a Certified Nursing Assistant course with at least 1 year of work as a nursing assistant.
- A valid and unrestricted driver's license.
- Heart Saver CPR or BLS certification within 6 months of hire.
- Experience with electronic health records is preferred.
Benefits: Salary $17.03 - $19.57 per hour, plus benefits package including medical, dental, and vision insurance.
