Your key responsibilities will include:
- Collaborating with social work and support service professionals in assessing the needs of individuals and families with developmental disabilities
- Developing and implementing personalized care plans and social support services
- Participating in providing individual and group counseling or therapeutic interventions
- Engaging in outreach and advocacy efforts within the community to raise awareness and support for individuals with developmental disabilities
- Contributing to the measurement and evaluation of the impact of support services on individual and family well-being
