Responsibilities
As a Clinical Team Lead, you'll coordinate hospice services according to acceptable standards of practice and company procedures. You'll review and approve patient information submitted by licensed professionals during various visits and facilitate relationships between physicians, referral sources, patients, caregivers, and employees.
Qualifications
- You must be a graduate of an approved school of professional nursing.
- You must be licensed in the state where you practice or meet board of nursing rules for nurse licensure compact.
- You should have two years of nursing experience and one year of experience in home health or hospice.
- You must demonstrate knowledge of federal, state, and local laws and regulatory guidelines governing home care operations.
- You should possess basic technology skills, including mobile device operation.

- You must maintain a valid state driver license.
- You must keep automobile liability insurance as required by law.
- You must have dependable transportation in good working condition.
- You must be able to drive safely in all weather conditions.
