About the Job:

">Responsibilities:
">- Prepare and evaluate assessments to identify patient needs and develop personalized care plans.
- Develop and implement social work service plans to ensure seamless transitions and optimal outcomes.
- Perform case management, coordination, and casework intervention to support patients and their families.
- Assist with administration, development, implementation, and evaluation of social work programs and policies.
- Participate in performance improvement initiatives and implement Quality Improvement processes.
Qualifications:
">- A master's degree in social work is required, with a license to practice in Hawaii.
- At least one year of social work experience is preferred.
- Demonstrated ability to network and collaborate with community agencies.
- Basic life support or CPR/AED certification is required.
Benefits:
">- This is a full-time role, with a 40-hour workweek.
- Working hours are from 08:00 AM to 04:30 PM, Monday to Friday, with occasional weekend coverage.
