Schedule: PRN Mon-Fri (8 AM - 5PM) - 1 day per week
Beaumont Home Health is seeking a compassionate and dependable LPN to provide skilled nursing care to patients in their homes.
What You'll Do
- Provide skilled nursing care according to physician orders and RN care plans
- Monitor patients and report changes to the RN
- Administer medications and treatments as prescribed
- Educate patients and families on care and disease management
- Complete accurate, timely documentation
- Communicate with patients, families, and the care team
- Participate in on-call responsibilities, meetings, and training
- Travel to patients' homes within the assigned service area
- Current LPN license in the state of employment
- 1 year of acute care LPN experience required
- 2 years preferred; home health experience a plus
- Strong communication, organization, and clinical judgment
- Compassionate and patient-focused
- Valid driver's license and auto insurance
