Essential Duties And Responsibilities
- Perform accurate assessment and treatment planning through evaluation of clinical symptoms, development and personality disorders, psychosocial stressors, and global function
- Provide effective counseling/case management services to patients
- Coordinate care of patients with healthcare providers, community agencies, specialists
- Coordinate and plans with outside agencies and home care nursing staff for post-discharge care
- Maintain timely, accurate documentation on all medical records in compliance with established guidelines and policies
- Demonstrate emotional resilience and stability
- Demonstrate excellent communication skills with patients and staff
- Capable of working with children and adults
- Perform other related duties as assigned by management
- Travel to various BJHCHS, Inc. medical center locations as needed and directed by the supervisor.
- This job may have supervisory responsibilities specifically for individuals with an LISW-CP. This need is dependent upon the needs of the department. This will be determined by the department supervisor.
- Master’s Degree in Social Work
- Current SC license as LISW/LCSW/LMSW
- A minimum of 3 years clinical experience providing direct services to adults, children, adolescents and families
- Minimum of 2 years case management and counseling experience
- Current knowledge of evidence-based models for mental health and substance abuse treatment
- Ability to deliver validated evidence-based therapy services to patients
- Able to develop treatment plans and document patient contact and progress
- Possesses a valid South Carolina driver’s license and clean driving record
