PURPOSE AND POSITION SUMMARY
This Family Support Specialist provides case management services to families in a Permanent Supportive Housing program while also offering support to children’s programming. Residents of the PSH program are experiencing poverty, disabling conditions, and homelessness, and this position provides them with supportive services to obtain and maintain housing. This Family Support Specialist empowers the entire household, including children, with skills for greater independence in the areas of goal setting, parenting, finance, employment, education, and housing stability. The Family Support Specialist is responsible for the proper assessment of the family’s needs, providing individual and group interventions, collaborating with other providers, and evaluating each resident’s progress toward goal attainment. The Family Support Specialist represents the professionalism and dedication that is associated with Beatitude House & Ursuline Sisters Mission.
ACCOUNTABILITY
The Family Support Specialist reports to the Support Service Coordinator and collaborates with the Children’s Program Coordinator.
RESPONSIBILITIES
To ensure the mission and purpose of Beatitude House is accomplished, this Support Specialist will:
- Carry out all policies and procedures of Beatitude House
- Establish positive partnerships with area resource providers
- Abide by the code of professional ethics
- Participate in staff meetings, public relations activities, and professional development as requested
- Prepare Incident Reports on situations that may have legal and/or organizational ramifications
- Answer phones and provide additional site coverage/staffing as requested
- Carry the on-call emergency phone as requested
- Stay updated on all government and grant regulations
- Provide support services to other agency programs as needed
- Provide any other services as requested by the Support Service Coordinator and/or Director
To carry out housing stability planning with residents, this Support Specialist will:
- Schedule and meet with the residents regularly in the office, the shelter, and their homes
- Facilitate group meetings with residents and provide education on basic life skills
- Develop goals with residents and document progress using the Housing Stability Plan
- Implement evidence-based interventions with residents, including harm reduction, motivational interviewing, and strengths-based approaches
- Assist residents with removing barriers to independent housing
- Provide information, education, interventions, and referrals related to the needs of the children in the household and coordinate with children’s programming
- Provide referrals and coordinate services with other providers while monitoring involvement
- Document goals, interventions, and accomplishments in records and databases
- Maintain current, up-to-date documentation of all case-related activity
- Assist with annual recertifications, ensuring all verifications/paperwork are collected
- Collaborate with other staff on event and program planning and facilitation
- Provide transportation for participants, as needed
To facilitate a resident’s transition to ongoing permanent housing and ensure continuity of care, this Support Specialist will:
- Continually assess when support is no longer needed to maintain housing
- Assist resident with appropriate housing referrals and ongoing community resource referrals
- Complete exit documentation and enter into databases
- Continue supportive contact for up to six months after a household’s exit
To support the well-being of children participating in agency-related programs, the Family Support Specialist will:
- Facilitate individual and family sessions in the home, office, or community
- Support parent-child communication, problem solving, and conflict resolution
- Provide educational information and resources to caregivers on parenting, child development, nutrition, self-esteem, safety, etc.
- Assess the family's needs across areas, including housing, healthcare, education, food access, legal, parenting, and family dynamics
- Refer and connect families to community resources such as counseling, tutoring, health care, child development, and other needed services
- Develop, implement, and monitor individual goals and stability plans, and document outcomes
- Coordinate with internal teams and programs as well as local agencies, mentors, and tutors to enhance client support
- Accompany children and families to appointments as needed
- Provide crisis management and support
- Keep up-to-date documentation and complete incident reports as needed
- Perform all other duties as assigned
To carry out the day-to-day operations of the Children’s Program in groups and our community, the Family Support Specialist will:
- Maintain an updated client/family roster
- Participate in agency-wide events and special projects as requested
- Assist in planning and facilitating age-appropriate children’s activities, socialization groups, enrichment events, family events, holiday parties, summer camps, and programming during adult-centered groups
- Provide after-school academic support and enrichment opportunities to children multiple times throughout the week
- Ensure compliance with Child Abuse Prevention Policies during programming
- Oversee volunteers or interns who assist with programming as requested
- Provide transportation to children or families attending group events
- All other duties as assigned
QUALIFICATIONS
- Bachelor’s degree in social work, child development, education, or related field preferred
- Minimum 2 years of experience working with children and families in poverty
- Strong verbal and written communication skills
- Experience in individual and group work, including case management and facilitation
- Knowledge of trauma-informed care and community-based resources
- Cultural sensitivity and the ability to work effectively with diverse populations

- Ability to work flexible hours, including some evenings and weekends
- Valid driver’s license and vehicle insurance to transport self and clients
· Ability to work independently, yet seek supervision when necessary
· Dedicated to ongoing personal and professional growth
- Proficiency in crisis management, documentation, and technology systems
- Physically able to move about facilities, ascend/descend stairs, and occasionally lift and carry up to 30 lbs.
