Join our team as a Clinical Liaison where you’ll play a key role in connecting with referral partners and prospective patients. You’ll help ensure a steady flow of appropriate referrals, support census goals, and guide patients through the admission process alongside hospital and Center staff.
Experience
- Proven background in Skilled Nursing, specifically in screening patients for potential admissions.
- Familiarity with a variety of Hospital EMR systems, including Allscripts, 4Next, NaviHealth, and Epic.
- Responsible for overseeing the full scope of the referral and admission process from start to finish.
- Skilled in receiving and evaluating referrals to determine clinical appropriateness for hospital admission.
- Collaborate closely with hospital teams to ensure all components of the admission process are completed thoroughly and accurately.
