As an LPN, you'll play a vital role in promoting the health and well-being of the individuals we serve by:
- Monitoring resident health concerns through daily follow-up and routine in-person visits
- Administering medications, injections, and vaccinations as ordered
- Completing nursing assessments, diabetic foot assessments, AIMS assessments, and Health Care Appraisals (HCAs)
- Coordinating medical care, appointments, hospital admissions/discharges, medication changes, and physician orders
- Monitoring medications for accuracy, interactions, side effects, and compliance
- Supporting psychiatric, medical, dental, and vision care coordination
- Providing hands-on medical skills training for Direct Support Professionals
- Participating in interdisciplinary care team meetings
- Educating staff and individuals on medication management, infection prevention, health maintenance, and medical equipment
- Serving as a healthcare liaison for families, guardians, providers, and care teams
- Auditing electronic health records (EHR) and electronic medication administration records (eMAR)
- Assisting with policy development, infection control initiatives, controlled substance destruction, and medical on-call rotation
- Triaging medical concerns and supporting continuity of care
- Performing additional duties as assigned
Required Qualifications
- Current Licensed Practical Nurse (LPN) license in the state of practice
- Minimum 2 years of nursing experience
- Valid driver's license with reliable transportation for travel between residential homes
- Ability to pass a criminal background check
- Current required certifications and mandatory training
- Strong English communication skills (written and verbal)
- Eligibility to work within Beacon programs in accordance with state and federal regulations
- Strong clinical nursing knowledge and sound judgment
- Excellent communication and relationship-building skills
- Ability to collaborate with multidisciplinary teams
- Experience managing multiple priorities in a fast-paced environment
- Strong computer and documentation skills
- Ability to de-escalate challenging situations with professionalism and compassion
- Passion for providing person-centered care
