Registered Nurse (RN) – Patient Care Services
Location: Community Hospital of Bremen | Department: Med/Surg | Schedule: PRN
About the Role:
As a Registered Nurse reporting to the Manager of Patient Care Services, you’ll deliver high-quality, compassionate nursing care while working collaboratively across interdisciplinary teams. You will assess patient needs, develop individualized care plans, and advocate for patient well-being, all while upholding our mission to deliver outstanding care, inspire health, and connect with heart.
What You’ll Do:

Deliver safe, evidence-based nursing interventions aligned with best practices and regulatory standards.

Develop and adapt personalized plans of care based on clinical assessment, patient preferences, and evolving needs.

Proactively address physical, emotional, and psychosocial risks, ensuring continuity of care across the patient journey.

Communicate effectively with patients, families, and care teams to ensure informed, patient-centered decision-making.

Maintain documentation that meets clinical and legal standards.

Serve as a mentor and role model for colleagues through collaboration, resource sharing, and support.
Qualifications:

Current RN license in the State of Indiana.

Bachelor of Science in Nursing (BSN) preferred. If not already held, must be obtained within 5 years of hire (for RNs hired after January 1, 2014), or obtain and maintain specialty certification.

Current BCLS (CPR) certification required; other certifications may be required per department.

Strong clinical assessment, critical thinking, and communication skills.
