Primary Responsibilities

• Working in decentralized areas alongside other health-professionals to promote safe medication use.  Areas of decentralization include Home Infusion Services, Hospital Medical Units, Mail Order and Pure Power Compounding.
• Monitoring medication therapy regimens for contraindications, drug-drug interactions, allergies, appropriateness of drug and dose for diagnosis and patient specific dosing parameters.
• Providing information to professional staff, patients and families which encourages safe, appropriate, cost-effective drug regimens and patient compliance.
• Providing clinical pharmacokinetics, TPN and anticoagulation dosing services for all appropriate patients.
• Pure powder pharmaceutical compounding for both human and veterinary patients.
• Assisting in department activities to meet regulatory agency standards and to improve patient safety and efficiency of the department.

Qualifications

• Education & Experience: Bachelor’s or Doctorate in Pharmacy (PharmD) from an accredited school.  A minimum of one year in hospital, home care or specialized compounding pharmacy experience is preferred.
• Licensure: Licensure or eligibility for licensure in the State of Indiana.

Knowledge & Skills: 

• Strong interpersonal and communication skills
• Knowledge of antibiotic stewardship principles along with the ability to apply pharmacokinetic dosing services to individualize antibiotic doses
• Knowledge of parenteral nutrition dosing and monitoring
• Knowledge of medication compounding, formulation, stability, and incompatibilities. 
• Demonstrates the analytical skills necessary to detect potential drug problems
• Knowledge to assist other healthcare providers in appropriate drug dosages and selection

Work Environment & Physical Demands

• Ability to work efficiently under pressure.
• Extended periods of computer screen time.
• Some lifting and bending are required.
