Primary Responsibilities
- Working in decentralized areas alongside other health-professionals to promote safe medication use. Areas of decentralization include Home Infusion Services, Hospital Medical Units, Mail Order and Pure Power Compounding.
- Monitoring medication therapy regimens for contraindications, drug-drug interactions, allergies, appropriateness of drug and dose for diagnosis and patient specific dosing parameters.
- Providing information to professional staff, patients and families which encourages safe, appropriate, cost-effective drug regimens and patient compliance.
- Providing clinical pharmacokinetics, TPN and anticoagulation dosing services for all appropriate patients.
- Pure powder pharmaceutical compounding for both human and veterinary patients.
- Assisting in department activities to meet regulatory agency standards and to improve patient safety and efficiency of the department.
Qualifications
- Education & Experience: Bachelor’s or Doctorate in Pharmacy (PharmD) from an accredited school. A minimum of one year in hospital, home care or specialized compounding pharmacy experience is preferred.
- Licensure: Licensure or eligibility for licensure in the State of Indiana.
Knowledge & Skills:
- Strong interpersonal and communication skills
- Knowledge of antibiotic stewardship principles along with the ability to apply pharmacokinetic dosing services to individualize antibiotic doses
- Knowledge of parenteral nutrition dosing and monitoring
- Knowledge of medication compounding, formulation, stability, and incompatibilities.
- Demonstrates the analytical skills necessary to detect potential drug problems
- Knowledge to assist other healthcare providers in appropriate drug dosages and selection
Work Environment & Physical Demands
- Ability to work efficiently under pressure.
- Extended periods of computer screen time.
- Some lifting and bending are required.
