Duties & Responsibilities
- Provide comprehensive nursing care to patients in their homes, ensuring adherence to individualized care plans.
- Conduct thorough patient assessments, including vital signs, medical history, physical examinations, and physiological evaluations to determine care needs.
- Administer medications via IV infusion, injections, and other routes following aseptic technique and safety protocols.
- Perform wound care, catheter management, ostomy care, and other specialized nursing procedures as needed.
- Collaborate with interdisciplinary teams to coordinate care, discharge planning, and optimize patient outcomes.
- Educate patients and families on disease management, medication adherence, nutrition, and lifestyle adjustments.
- Utilize EMR (Electronic Medical Records) systems for accurate, efficient documentation and communication of patient information.
- Prepare clinical and progress notes for each patient visit and summaries of care conferences in a timely manner per agency policy.
- Complete and submit OASIS assessments, reassessments, transfers, resumptions of care, discharges, and significant change in condition documentation in accordance with agency-defined time frames.
- Ensure compliance with HIPAA regulations and maintain confidentiality of all patient records.
Basic Qualifications
- Valid Registered Nurse (RN) license in the state of California.
- Minimum of two (2) years of nursing experience.
- Strong knowledge of nursing principles, practices, and medical terminology.
- Valid driver's license and current auto insurance.
Preferred Qualifications
- Experience with home health care or community-based nursing.
- Proficiency in using EMR/EHR systems; experience with WellSky (Kinnser) is highly preferred.
- Familiarity with ICD-10 coding and OASIS documentation.
- Background in medical-surgical, geriatrics, critical care, or acute care settings.
- Experience working with diverse populations, including individuals with chronic illnesses or developmental disabilities.
- Skilled in advanced procedures including catheterization, tube feeding management, laboratory specimen collection, and ventilator care.
