one of the following areas: Social Work, Sociology, Psychology, Counseling, or Criminal
Justice.
Experience: Required - 2 years of experience providing direct case management services. A
Master’s degree in a social service-related field may be used to substitute for one year of
experience. Required- 1 year of experience providing intensive in home- based interventions.
Summary: The Case Manger is responsible for using sound professional judgement and best
practices to provide case management to the service population including crisis intervention as
needed while maintaining compliance with established state and federal standards.

Essential Functions:
- Provides intensive home-based intervention case management services using a Trauma- Informed
assigned caseload.
- Provide crises intervention responding promptly and professionally to crises situations with
independent judgement and discretion.
- Develop, monitor, and review Individual Treatment Plans designed to increase strengths,
- Provide linkages to community resources for clients to assist with enrollment in public
critical documents such as birth certificates and SS/ID cards and transportation needs.
- Assist in locating appropriate housing for clients upon exit from the Transitional Housing
- Mandated reporter - Report any suspected abuse or neglect to the program supervisor and all
- Attend and participate in; staff meetings, in-service trainings and UCAP events.
- Participate in weekly face to face or phone supervision with the supervisor.
- Frequent travel to assist clients with community resource connections as needed.
- Meet all deadlines required by program supervisor and state/federal partners.
- Meet all federal and state regulatory guidelines and standards that are applicable to this
- Maintain a caseload in accordance with agency policies and procedures and licensing and
- Provide services to youth enrolled in the Transitional Housing Program.
- Complete training and received ongoing education on skills necessary for conflict
- Conduct follow up calls and/or visits to young adults in crises situations and complete safety
- Compile, prepare, submit and maintain accurate case records, files, forms and additional
- Work flexible hours as needed including evenings, weekends and holidays when approved by
- Implement BCFS HHS safety protocols including evacuating with children and other staff in case
- Maintain confidentiality in all areas of the service population and program operations.
- Maintain BCFS HHS professional and ethical standards of conduct outlined in the BCFS employee
and community members and complying with the required dress code, at all times.
Credentials, Skills and Abilities:
- Maintain licenses and and certificates required for the including but not limited to:

- Pass a pre-employment and biennial criminal background and motor vehicle check
