Nurse Practitioner Job Description
Position Summary:
The role of the Advanced Practice Provider (APP), Associate Medical Director at BGHC is instrumental in delivering cost-effective, timely and high-quality health care services for the underserved patients who utilize BGHC services. This position is responsible for providing Acute Care and Chronic Care to BGHC patients presenting with a wide variety of conditions. This position oversees timely follow-up care for patients seen by the clinic’s volunteer providers, responsible for advanced level of clinical evaluation, clinical assessment, prescribing and monitoring medications and follow-up care for patients by working closely with Care Management nurses on referrals. This person will have excellent communication skills both written and verbal and maintain the highest standards of professional excellence in the implementation of policies and procedures.
Key Responsibilities:
1. Medical Leadership
- Effectively identify, evaluate, and address disease prevention and health promotion
issues of the population in practice while administering quality patient care.
- Work in an independent and interdependent relationship with members of the
medical staff, which allows for consultation, collaboration, or referral.
- Write Standing Orders, answer medical questions, and consult on Policies and
Procedures
- Supervise and manage staff, Patient Navigator, Nurse Case Manager, and Community
Health Worker.
- Manage volunteers for Immunizations, Hypertension Education, Diabetic Education,
Dietician, and others as needed.
- Serve as the laboratory contact to address abnormal labs that need urgent
evaluation and referral to urgent or emergent care follow up.
- Provide on-site training and support for volunteers engaging in patient care or
coordination for assigned clinics.
2. Patient Care
- Treat illnesses
- Prescribe medications and treatments and ongoing medication management. Order
lab and diagnostic tests in a cost-effective manner and interpret test results.
- Utilizes Telehealth options for patient care as appropriate/needed.
- Respond to telephone inquiries to provide medical care advice.
- Educate patients and/or families about preventive care, medical issues, and use of
prescribed medical treatments and/or medications.
- Maintain legible, accurate, and confidential medical records. Documents all medical
evaluations, diagnoses, procedures, treatment, outcomes, education, referrals and
consultations according to established standards.
3. Administrative (In the absence of a Clinic Director)
- Supervise and manage front desk tasks that impact total medical services, these
include schedules for volunteer staff at front desk, front desk tasks list, Spanish line,
evaluation of medical provider schedules, and coverage for volunteer nurses.
- OHSA/WSHA annual and monthly facility reviews.
- Safety meetings monthly.
- Facility Equipment maintenance
- Lab maintenance and supply ordering
4. Other
- Comply with federal and state regulations.
- Follow Board policies and directives.
- Ensure BGHC’s Health’s policies and procedures are upheld.
- Analyze new knowledge gained from conferences, workshops, professional literature, or
“hands-on training” and assimilates this knowledge into clinical practice.
- Work collaboratively with volunteers, nursing, and other staff.
- Attend to the physical, mental, spiritual, and social needs of all patients.
- Perform other duties as assigned.
Qualifications
Education
- Advanced Practice professional license and valid Washington State license.
- Maintain continuing educational state and certification requirements.
- Maintain current and valid CPR training and BLS certificate.
- Maintains current DEA license.
Experience and Training:
- Demonstrated knowledge of and ability to apply age specific principles of growth and
development and life stages to meet each patient’s needs.
- Has experience in treating and caring for individuals with Chronic Conditions.
- Able to use electronic medical record software – including input of new patients,
screening new patients, demographic data, scheduling of appointments, scanning in
documents, communication with staff.
- Experience in community health or public health setting.
- Comfortable creating and implementing policies, procedures, and standards of care.
- Highly organized with strong attention to detail.
- Good judgment in establishing priorities and problem solving.
License/Certification:
- Licensed to practice in the state as an Advanced Practice Provider; (ARNP/ PA-C)
- Current BLS (Basic Life Support) certification required at date of hire.
- Have national prescriptive drug privileges with a DEA number.
- Current Driver’s License and insurance required.
