Social Worker

For fifty years, Bayview Senior Services, (aka Bayview Hunters Point Multipurpose Senior Services) has responded to community needs and served as a catalyst for neighborhood activism and advocacy. Programs are designed to enhance and retain the health, quality of life, and culture of African American elders who remain as long-standing members of communities in San Francisco.

We are searching for a highly motivated Social Worker to join our team!

The Social Worker is responsible for providing comprehensive social work services to adult day health participants, their families, and community partners. This position conducts assessments, develops individualized care plans, provides counseling and care coordination, connects participants with community resources, and supports participants' health, independence, and overall quality of life through interdisciplinary collaboration.

Minimum Requirements

• Master of Social Work (MSW) degree from an accredited program.
• Current California licensure (LCSW preferred or as required by program regulations).
• Minimum of two years of experience working with older adults, individuals with disabilities, or in a health care or community-based setting.
• Valid California driver's license, reliable transportation, and proof of insurance.

Duties and Responsibilities

• Conduct intake and screening assessments to determine participant eligibility and appropriateness for services.
• Perform individual and group psychosocial assessments and develop personalized Individualized Care Plans (ICPs).
• Monitor participant progress and modify care plans as needed to address changing needs and goals.
• Provide advocacy services to ensure participants' rights are upheld and assist individuals in accessing appropriate community resources.
• Coordinate care with physicians, therapists, families, and other healthcare providers to support participant well-being.
• Research and arrange referrals to community resources, self-help groups, and state or local government agencies as appropriate.
• Provide individual and group counseling, including family and caregiver support, as needed.
• Maintain accurate, timely, and confidential documentation of participant assessments, care plans, progress notes, and related records.
• Participate in team meetings, interdisciplinary case conferences, staff training, quality improvement initiatives, and community outreach activities.
• Support the overall mission of Bayview Senior Services by collaborating with staff across programs and performing other duties as assigned.

Knowledge, Skills, and Abilities

• Strong knowledge of psychosocial assessment, counseling techniques, and care planning.
• Knowledge of aging services, community resources, public benefits, and government assistance programs.
• Ability to communicate effectively with participants, families, caregivers, healthcare professionals, and community partners.
• Excellent organizational, documentation, and case management skills.
• Strong verbal and written communication skills.
• Ability to maintain confidentiality and exercise sound professional judgment.
• Demonstrated ability to work independently and collaboratively as part of an interdisciplinary team.
• Bilingual skills (English/Korean, English/Chinese, or other languages) are preferred.
• Commitment to providing compassionate, person-centered services that promote participant independence and quality of life.
