Position Overview:
We are seeking Family Nurse Practitioners (FNPs), Adult Nurse Practitioners (ANPs), Adult-Gerontology Nurse Practitioners (AGNPs) and Physician Assistants (PA-C) to join the NYIA Independent Practitioner Panel in New York. This panel is responsible for issuing physician orders for individuals who have completed a Community Health Assessment and are eligible for community-based long-term care services and supports.
As a practitioner, you will conduct follow-up in-home clinical visits for individuals who have undergone assessments. Comprehensive virtual training is provided and compensated.

Albany, Allegany, Cayuga, Chautauqua, Chemung, Chenango, Clinton, Cortland, Delaware, Franklin, Herkimer, Jefferson, Lewis, Livingston, Madison, Monroe, Montgomery, Niagara, Oneida, Onondaga, Orange, Oswego, Otsego, Rensselaer, Schuyler, Seneca, Schenectady, Steuben, Tioga, Tompkins, Wayne.
Compensation:
- Hourly Rate: $115.00 per hour
Visit Structure (90 minutes per case):
- 30 minutes – Reviewing RN documentation
- 30 minutes – Conducting the clinical visit
- 30 minutes – Charting
Requirements:
- Board certification and personal medical malpractice insurance
- Active NYS Medicaid ID number
- Ability to conduct in-person visits in the community
- Must be conflict-free, with no affiliations to:
- Managed Long Term Care (MLTC)
- Licensed Home Care Services Agencies (LHCSA)
- Certified Home Health Agencies
- Insurance companies or Institutional Special Needs Plans (ISNPs)
- Minimum 2 years (3,600 hours) of relevant experience
Available Schedules:
- Monday – Friday: 8 AM – 7 PM
- Saturday & Sunday: 10 AM – 6 PM
- Part-time schedules available; this is an in-person position in NYS. No remote work available.

Job Responsibilities:
- Review and provide recommendations on consumer assessments for long-term care services and supports
- Assess completed Community Health Assessment tools in the NYS Uniform Assessment system to assist in physician orders
- Stay informed about regulatory changes affecting assessment quality and validity
- Utilize clinical expertise to validate assessment outcomes
- Collaborate to monitor and address clinical quality and care gaps
