POSITION TITLE: Care Coordinator - Emergent Care (Pastor on Call, Hospital Visitation, Memorials, and Benevolence)
BAYSIDE PURPOSE: We exist to help people know God, Find Family, Live in Freedom, and discover their Purpose.
MINISTRY PURPOSE: Care Ministry exists at Bayside to offer help, hope, and support to people as they walk through various life experiences.
POSITION STATUS: Exempt
WORK SCHEDULE: Full-Time 40 hours: 5 days a week, Tuesday through Friday, and Sunday services.
REPORTS TO: Campus Care Director

PRIMARY RESPONSIBILITIES:
- Train and monitor full-time staff assigned to campus pastor on call rotation
- Oversee a team of volunteers serving to facilitate, document, and follow-up on hospital visits to members and their families
- Oversee a team of volunteers serving to facilitate, document, and follow-up on memorials for members and their families
- Identification, onboarding, and support of hospital visitation team volunteers and memorial team volunteers
- Vision casting for pastor on call staff, hospital visitation team, and memorial team
- Ongoing training, evaluation, and support to pastor on call staff, hospital visitation team, and memorial team
- Assess requests for benevolence assistance and make recommendations regarding referrals and/or direct support
- Development of care plans with members as needed
- Assisting campus care staff with a variety of care related requests as needed
- Report weekly to Campus Care Director
- Be knowledgeable about and maintain connections with local hospitals, in-patient facilities, and hospice care
- Be knowledgeable about and maintain connections with local funeral homes and funeral directors
- Be knowledgeable about and maintain connections with local community service organizations
- Purchase and maintain materials used to care for members and their families
- Assisting and troubleshooting of PCO and Google Drive for Leaders and Coaches
- Aiding leaders and coaches with care related issues
- Identification and compilation of helpful resources and tools for leaders and members
- Numerical Measures
- Increase in the number of members served (reported quarterly)
- Faithful and effective allocation of allotted budget (reported monthly)
- Spiritual Measures
- Documented investment of time and resources in spiritual care of leaders (reported quarterly)
- Documented investment of time and resources in personal spiritual development (reported annually)
- Ministry Efficacy
- Consistently share leader stories of healing and breakthrough (reported weekly)
- Consistently share member testimonials of life change (reported weekly)
- Consistently demonstrate continuity of care where members are offered next steps (reported quarterly)
- Minimum of 3 years previous ministry leadership experience required
- Biblical literacy and proficiency in scriptural application required
- Demonstrated ability to lead and equip others required
- Documented training in care ministry and/or pastoral care
- Verbal and written communication skills required
- Basic computer skills required
- Proficiency in Google Workspace or Microsoft Office preferred
- Proficiency in Planning Center Online or other church management software preferred
- Prior experience managing budgets preferred
