Position Summary
We are seeking a compassionate, motivated, and skilled Physical Therapist (PT) to join our rehabilitation team at Bayshore Residence and Rehabilitation Center. The Physical Therapist is responsible for evaluating, planning, and implementing individualized therapy programs to maximize residents’ physical abilities, mobility, and independence while ensuring compliance with state and federal regulations, including those specific to Minnesota long-term care settings.
Key Responsibilities
- Evaluate residents’ functional abilities and develop individualized treatment plans to address physical impairments and improve quality of life.
- Provide direct physical therapy interventions, including therapeutic exercise, gait training, balance activities, and pain management.
- Document evaluations, treatment sessions, progress notes, and discharge summaries in accordance with facility, state, and federal requirements.
- Collaborate with an interdisciplinary team including physicians, nurses, occupational therapists, speech-language pathologists, and social workers to coordinate resident care.
- Educate residents, families, and caregivers on therapy programs, safety techniques, and strategies to maximize independence.
- Monitor residents’ progress and adjust treatment plans as needed.
- Adhere to ethical and professional standards as outlined by the Minnesota Board of Physical Therapy, facility policies, and CMS regulations.
- Participate in care plan meetings and quality improvement initiatives.
- Maintain compliance with HIPAA and infection control protocols.
- Graduate of an accredited Physical Therapy program.
- Current Physical Therapist (PT) license in the State of Minnesota (in good standing).
- Knowledge of federal, state, and local regulations related to skilled nursing facilities.
- Strong communication, teamwork, and organizational skills.
- Ability to work effectively with older adults and individuals with complex medical conditions.
- Prior experience in a skilled nursing facility or long-term care setting preferred, but not required.
- Ability to stand, walk, bend, lift, and move intermittently throughout the day.
- May be exposed to infectious waste, diseases, or conditions.
