Pay: Competitive Weekly Pay (Based on Experience)

Responsibilities: Provide skilled nursing care to hospice patients in homes, assisted living facilities, nursing homes, and inpatient settings. Complete comprehensive patient assessments and develop individualized plans of care. Manage pain and symptom control while promoting patient comfort and dignity. Coordinate care with physicians, hospice interdisciplinary teams, families, and caregivers. Educate patients and families regarding disease progression, medications, and end-of-life care. Complete accurate and timely electronic medical documentation (EMR). Participate in on-call rotation as required by the client.
QualificationsRequired: Active Louisiana Registered Nurse (RN) License, Minimum of one (1) year of recent Hospice RN experience, Current BLS Certification, Valid Driver’s License Reliable transportation and current auto insurance, Strong assessment, communication, and time management skills
Preferred: Hospice Case Manager experience, HCHB documentation experience Experience with electronic medical records (EMR)
