General Summary:
Provide care, coordinate, and support Phases I, II and III for Cardiac and Pulmonary Rehabilitation, and Supervised Exercise Therapy for Peripheral Artery Disease patients. Collaborate with a team of Exercise Physiologist, Cardiac Rehabilitation Nurse, and Registered Respiratory Therapist to implement and design exercise programs tailored to the needs of patients recovering from Cardiac, Pulmonary and Vascular conditions. The expertise will help improve the patient's cardiovascular functions, manage symptoms, educate about risk factor modification, and improve quality of life.

General Summary:

Responsibilities:
- Referral/Enrollment/Registration/Scheduling: Demonstrates ability to competently perform complete referral, scheduling, and registration workflow processes for cardiac and pulmonary rehabilitation program services.
- Exercise Prescriptions: Collaborates with cardiopulmonary rehabilitation team members to develop a safe, effective exercise prescription (frequency, intensity, time, type) for the patient based on the initial assessment, medical history, test results, physician orders, limitations, and individualized goals.
- Walk Studies and Diagnostic Testing: Performs complete and accurate 6 and 12-minute walk studies and diagnostic testing but not limited to exercise-induced asthma studies and metabolic studies.
- Individual Treatment Plan (ITP): Works collaboratively with cardiopulmonary rehabilitation team members, medical director, and patient in developing and documenting the Individual Treatment Plan (ITP) in consideration of physician’s orders, medical history, assessments (Functional, Nutritional, Psychosocial), limitation and patient goals.
- Direct Patient Care: Collaborates with cardiopulmonary rehabilitation team members to help patients execute and accomplish their Individual Treatment Plan (ITP)/ exercise prescription by instructing, encouraging, monitoring, and assisting them in performing their prescribed physical activities.
- Patient Intakes/Orientations: Assist and support the performance of comprehensive patient initial intake and assessment (Functional, Nutritional, and Psychosocial Assessments) to gather relevant patient needs in collaboration with patients, family members, and cardiopulmonary rehabilitation team members.
- Patient education, counseling, and coaching: Identifies risk factors and their possible impact on disease and rehabilitation. Understand basic educational principles and theories of learning as well as those factors that affect learning and utilize this to develop an educational program with an interdisciplinary approach for each patient.
- Responsible for assisting in AACVPR certification and recertification in Cardiac and Pulmonary Rehabilitation.
- Completes competencies and mock codes in accordance with the requirements of AACVPR.
- Represent the department in providing educational information to the community and other services in the area.
- Mentor, orient, and assist with training department staff and interns.
- Safety and Emergency Procedure: Demonstrates the ability to collaborate with cardiopulmonary rehabilitation team members to perform all aspects of patient care in an environment that optimizes patient safety and reduces the likelihood of medical/healthcare errors. Initiates and assists with emergency care per policy and procedures.
- Outcome and Performance Improvement Measures: Demonstrates the ability to collaborate with cardiopulmonary rehabilitation team members to assess outcome and performance improvement measures in cardiac and pulmonary rehabilitation programs.
- All other duties as assigned within the scope and range of job responsibilities.
- Education: Bachelor Degree
- Exercise Physiology
- Or Bachelor of Science in Exercise Science, Kinesiology or other related field. Emphasis in Cardiac or Pulmonary Rehabilitation.
- Credential(s): Basic Life Support
- Experience: Required: One year of experience in Cardiac/Pulmonary Rehabilitation and/or Skilled internship in Cardiac Rehabilitation.
Preferred Education, Credential(s) and Experience:
- Education: Master Degree Exercise Physiology
- Credential(s): Medical Exercise Specialist Advanced Cardiovascular Life Support ACSM Exercise Specialist certification and/or ACE Exercise Specialist certification, CCRP certification from AACVPR
